    This service provides community mental health navigation services across 3 Primary Care Networks (PCN’s) in the Grimsby area. Staff are based out of GP Practices and support individuals with mental health issues to access and engage with a range of health care services. This programme is delivered in partnership with Navigo. People can't self refer, referrals come from GPs and are triaged by a clinical lead before being handed over to the Navigator. The service covers 5 primary care networks in NE Lincs - Meridian, Freshney Pelhan, Panacea, Apollo and Genesis.
