East Devon Community Outreach

Our East Devon Community Outreach service are a team of Mental Health Recovery Workers providing mental health support to those in their own homes in the community. We provide weekly support to those with a support package through Devon Partnership Trust NHS and Devon County Council. We provide recovery based support to meet the needs of the individual.

Rethink Mental Illness deliver talking therapies and specialist mental health through the gate support for individuals residing in HMP Humber. Talking therapies are delivered in line with Step 1 – 2 of the NHS Talking Therapies (formerly IAPT) model and include interventions from Wellbeing Coaches, PWP’s and Counsellors. In addition dedicated mental health Care Navigators, provide support for those being released into the community to ensure mental health is maintained during this time. Our provision in HMP Humber is delivered as a subcontract with Tees, Esk and Wear Valley Foundation Trust and together we provide an integrated and holistic mental health provision. Individuals can self refer or be directly referral by a health professional.

Access Community Mental Health (Wiltshire)

The Rethink Access Community Mental Health Wiltshire Service is closing as of the 31st March 2025. • From 1 April 2025, some community mental health services in B&NES, Swindon and Wiltshire (BSW) will be provided by a new voluntary sector partnership called the Mental Health and Wellbeing Partnership (Alabaré, Missing Link, Nilaari, Second Step). This replaces the Rethink Access Community Mental Health Wiltshire Service that Rethink provides. • Guided by principles of equity, accessibility and coproduction, we are collaborating with providers across the system to ensure service continuity through this transition phase. • Together we will work to provide integrated care and the best possible outcomes for people across BSW. In addition our Care Navigator supports individuals with the lead up to release, and post release to manage their mental health needs and successfully resettle in the community. Our provision in HMP Altcourse is delivered as a subcontract with Merseycare NHS Foundation Trust and together we provide an integrated and holistic mental health provision. North Devon Supported Living Services - Gloster Road

Gloster Road is a supported living service providing accommodation with support commissioned by Devon Partnership Trust NHS and Devon County Council. We have a friendly, mixed-gender team of Mental Health Recovery Workers who provide 1:1 support to our clients, as well as hosting various group activities and outings. This service is a support and enabling, mental health specific service, providing recovery support to our clients with a goal of moving on to independence within 2 years. We also offer spaces on our Community Outreach team so that clients can continue to be supported in their own homes where needed, with a view to gradually reduce support hours as support needs change. Our staff teams are interlinked, and support planning is carried out in the same way, meaning consistency from one service to the other.

Salisbury Supported Housing

Salisbury Supported Housing, Sydney House is a large Victorian house on the edge of Salisbury City Centre. Sydney House is a low to medium-supported housing scheme for adults with mental health issues. Service users receive up to 10 hours of support a week from Mental Health Recovery Workers to help them work towards moving independently into the community through Homes4Witlshire. Specific person-centered goals tailored to the individuals’ needs are worked towards to support moving independently within a 2-year tenancy. Nine service users have their own rooms and share a large communal area, kitchen, and laundry room. Staff and service users arrange activities in the house, and out in the community. There is a warm and friendly atmosphere in the service where people who have been unwell are supported to gain the skills needed to have a fulfilling and meaningful life. We offer hope and compassion for a positive future for those we work with. The service offers between six- and twelve-weeks outreach support at two hours a week where needed after moving on. Service hours are 9 – 5pm weekdays and bank holidays.

Grimsby Care Navigation

This service provides community mental health navigation services across 3 Primary Care Networks (PCN’s) in the Grimsby area. Staff are based out of GP Practices and support individuals with mental health issues to access and engage with a range of health care services. This programme is delivered in partnership with Navigo. People can't self refer, referrals come from GPs and are triaged by a clinical lead before being handed over to the Navigator. The service covers 5 primary care networks in NE Lincs - Meridian, Freshney Pelhan, Panacea, Apollo and Genesis.

Mendos

MENDOS provides support, information and advocacy for people aged 18+ who live in Brighton and Hove, who have mental health challenges who are involved with the criminal justice system. We work with people who are on probation, going through the courts, or have recently been released from prison or discharged from secure psychiatric hospitals. MENDOS can help you get back into the community after discharge from probation services or prison. Through joint working with other partners and services locally. We can provide a range of support.
